When using the ISS or Shuttle telephone, it does not take much longer to make a connection than it does here on Earth. Connections are not a problem. The ground controllers are in constant communication— with the exception of some short periods of time—and whenever the astronaut wants to talk to the ground, he says something like "Houston, Discovery" and Houston answers "Discovery, Houston" and the communication begins.
